From the restaurant Los Doce Apóstoles although I have the address, I haven't found much information about the menu, its interiors, nothing at all. According to its fiscal data, I can deduce that it opened in 1987 and was located in Plaza República de Ecuador in Madrid.

What I have been able to trace is that the engraving that appears on the box is very specific, it belongs to one of the pre-Romanesque paintings of the Church of San Quirico de Pedret, specifically the famous "orant" within the circle with the bird above. It seems to me a very particular choice of image, I would say that perhaps the designer of the box or the owner of the restaurant probably had artistic or architectural training, an interest in Catalan Romanesque art, and/or some connection to Catalonia.
